How insurance treats Wegovy® and Zepbound® for weight management, where Medicare stands, what FSA and HSA accounts can pay for, and how to appeal a denial.

## Coverage depends on the plan, not the medication

Wegovy® and Zepbound® are FDA-approved for chronic weight management, but approval does not make a plan pay for them. Each employer or marketplace plan decides whether anti-obesity medications are on its formulary at all, and many still exclude them outright. Plans that do cover them almost always require prior authorization, which means the prescriber has to document that the patient meets the plan's criteria before the pharmacy can fill the prescription.

The criteria vary by plan, but the ones that come up most often are a body mass index of 30 or more, or 27 or more with a weight-related condition such as high blood pressure or type 2 diabetes, a documented attempt at diet and exercise, and sometimes a requirement to try a lower cost medication first. Coverage can also be withdrawn at renewal. Before assuming anything, ask the plan for its current formulary and its prior authorization criteria in writing.

Personalized compounded medications differ from commercially available brand-name products in formulation, concentration, excipients, preparation methods, and other characteristics. Accordingly, clinical data from studies involving brand-name products should not be assumed to apply to personalized compounded formulations.

## Where Medicare and Medicaid stand

Federal law has long excluded drugs used for weight loss from Medicare Part D coverage. Part D plans can cover Wegovy® when it is prescribed to reduce the risk of major cardiovascular events in adults with established heart disease and obesity or overweight, which the FDA approved in March 2024, and Zepbound® when it is prescribed for moderate to severe obstructive sleep apnea in adults with obesity, approved in December 2024. The weight management indication on its own remains outside Part D. Federal pricing arrangements announced in late 2025 were changing who pays what during 2026, so a Medicare member should confirm current coverage with their plan rather than rely on a general rule. Medicaid coverage is decided state by state.

## Compounded programs and insurance

Personalized compounded semaglutide and tirzepatide are not FDA-approved products, and insurance plans generally do not cover them. Programs such as Bodi Envi are self-pay. The published Bodi Envi price for Personalized Compounded Semaglutide starts at $155 per month billed monthly and goes down to $125 per month on a 12-month prepaid plan, and Personalized Compounded Tirzepatide starts at $229 per month and goes down to $175 per month on a 12-month prepaid plan. Every plan price includes the provider evaluation, the medication, supplies, shipping, ongoing provider access and dose changes. For comparison, the manufacturer's published price for Zepbound® starts at $1,399 per month and for Wegovy® at $1,699 per month without insurance. Bodi Envi does not require insurance.

## FSA and HSA accounts

IRS Publication 502 treats the cost of a weight loss program as a medical expense when it is for the treatment of a specific disease diagnosed by a physician, and it names obesity as an example. Prescribed medications and the fees for the medical visits that go with them are also medical expenses. That is the test for whether an expense is eligible. Whether a particular program can accept an FSA or HSA card at checkout is a separate question that depends on how the merchant is set up, so ask the program directly, pay another way if the card is declined, and keep the itemized receipt and any letter of medical necessity for a reimbursement claim. Publication 502 is explicit that diet foods and gym memberships for general health do not qualify. Bodi Envi accepts HSA and FSA accounts.

## What to do when a prior authorization is denied

1. Read the denial letter for the stated reason. It will name a criterion that was not met or not documented, and that is what the appeal has to address.
2. Ask the prescriber to file an internal appeal with a letter of medical necessity. The letter should quote the plan's own criteria and show how the record meets them, with the BMI, the weight-related conditions and the dates of any prior programs.
3. Note the deadlines. Under federal rules a plan has 30 days to decide a standard pre-service appeal, and 72 hours for an urgent one.
4. If the internal appeal fails, request an external review by an independent reviewer. The plan's denial letter has to explain how, and HealthCare.gov describes the process for plans it covers.
5. If the plan simply excludes anti-obesity medications, an appeal cannot add a benefit the plan does not have. That is the point at which many people compare the full cash price of the brand against a compounded program, and talk to a licensed provider about which option is appropriate for them.

## Frequently asked questions

### Does insurance cover compounded semaglutide or tirzepatide?

Generally, no. They are not FDA-approved products and plans do not list them on formularies. Programs that provide them are self-pay. Personalized compounded medications differ from commercially available brand-name products in formulation, concentration, excipients, preparation methods, and other characteristics. ### Can I use my HSA for a GLP-1 weight management program?

The IRS treats a weight loss program prescribed for a diagnosed condition such as obesity, and the prescribed medication, as medical expenses. Whether a given program can take the card at checkout depends on the merchant, so ask first and keep receipts for reimbursement. Bodi Envi does accept FSA and HSA accounts for payment. ### Why was my prior authorization denied when my BMI qualifies?

Most often because something was not documented: the weight-related condition, a prior lifestyle program, or a required step therapy. The denial letter names the gap and the appeal addresses it. ### How long does an appeal take?

A standard internal appeal must be decided within 30 days for a pre-service request. An external review is a further step with its own timeline, described in the denial letter. Personalized compounded medications differ from commercially available brand-name products in formulation, concentration, excipients, preparation methods, and other characteristics. ### Does Medicare cover Wegovy® or Zepbound®?

Part D can cover Wegovy® for cardiovascular risk reduction and Zepbound® for obstructive sleep apnea. Weight management on its own has been excluded by statute. Check with the plan, because arrangements changed during 2026.
