How to read the brand rows. Lilly's pricing pages define a list price as the wholesale acquisition cost, the price at which Lilly sells to wholesalers, and say plainly that what a patient pays largely depends on their insurance plan. Both pages define a monthly fill as 28 days, and the Mounjaro® page defines it as four pens. The Zepbound® list figure is a range because the product comes in several presentations, and the page states the wholesale figures were accessed on January 1, 2026. The direct to patient row is a different kind of price, which Lilly describes as the price at which it sells directly to cash paying patients, and it is covered in its own section below.

Bodi Envi's price is the same at every dose. With Lilly's direct to patient pricing it is not, and that difference turns out to matter more than the headline numbers.

## What drives the difference

The gap between $229 and a four figure list price is not the same product at two prices. It is two different things, and it is worth being exact about what each one is.

Zepbound® and Mounjaro® are FDA-approved. Lilly ran the clinical trials, submitted the data and received approval for specific uses, which the prescribing information for each product sets out. Lilly manufactures them, sets the list price, and runs the savings and direct to patient programs described later. Every element of the product, from the concentration in the pen to the pen itself, was reviewed before it went on sale.

Personalized Compounded tirzepatide is prepared by a licensed pharmacy for one patient, based on that patient's prescription. The FDA's human drug compounding page defines compounding as combining, mixing or altering ingredients of a drug to create a medication tailored to the needs of an individual patient, and it states that compounded drugs are not FDA approved, which means the agency does not review their safety, effectiveness or quality before they are marketed. According to the FDA's compounding questions and answers, day to day oversight of state-licensed pharmacies that are not registered as outsourcing facilities, the 503A pharmacies Bodi Envi's prescriptions go to, belongs primarily to state boards of pharmacy.

Bodi Envi states the consequence in its own words: compounded personalized tirzepatide is not FDA-approved and is not equivalent to FDA-approved products. The lower price reflects that difference, not a discount on the same thing, and it belongs in the decision alongside the money. The provider who reviews your assessment is the person to have that conversation with.

## What is inside Bodi Envi's price

The monthly figure buys a program rather than a vial, and Bodi Envi publishes what the program contains.

The price includes the online health assessment, review by a licensed healthcare provider, ongoing messaging with your care team through the patient portal, and the medication itself when a provider prescribes it, shipped to your home. Clinical services for applicable programs are provided by Wasef Health, PC. Prescriptions for applicable GLP-1 programs are dispensed by VialsRx, a third party U.S. 503A licensed compounding pharmacy.

One published rule shapes the comparison more than any single figure: for applicable programs, pricing does not increase based solely on dosage strength. The Zepbound® prescribing information starts adults at 2.5 mg once weekly and allows increases in 2.5 mg steps after at least four weeks at each step, up to a maximum of 15 mg, so a patient following the approved schedule may move through several doses over months. At Bodi Envi that progression does not change the monthly price. The dose decision stays clinical.

Two limits apply and Bodi Envi states both. Medication is included only when a licensed healthcare provider determines treatment is medically appropriate and issues a prescription. And completing an assessment or paying for a program does not guarantee a prescription. The Plans page carries the current figures and the full terms.

## Lilly's direct to patient vials and savings programs

Lilly publishes prices below list for some patients, and because the numbers are specific they deserve to be repeated exactly as Lilly's own pages stated them when this guide was checked in September 2026. The pages carry their own terms and end dates, and they change.

According to Lilly's Zepbound® pricing information page, Zepbound® is available through self pay as a four count single-dose vial and as a single-patient-use KwikPen® in doses of 2.5 mg, 5 mg, 7.5 mg, 10 mg, 12.5 mg and 15 mg, with direct to patient pricing of $299, $399, $499, $699, $699 and $699 respectively. The page describes this as the price at which Lilly sells directly to cash paying patients and notes it is not the wholesale acquisition cost. Lilly's Zepbound® savings page repeats the same schedule as the regular price for a one month supply of the KwikPen® and adds that the 2.5 mg dose is a starting dose, not an approved maintenance dose.

For people with commercial insurance that covers Zepbound®, the pricing page says they may be eligible to pay as low as $25 for a one or three month supply of the single-dose pens, with government beneficiaries excluded and monthly and annual limits on the savings. The savings page adds an option for commercially insured patients whose plan does not cover the pen to pay as low as $499 for a one month fill, and states the savings card expires on December 31, 2026. It also describes a Medicare option for eligible Part D patients with obesity at no more than $50 per month, marked as not a guarantee of coverage.

The practical reading is this. At the 2.5 mg starting dose, Lilly's direct to patient vial price of $299 sits above Bodi Envi's $229 monthly price. At 5 mg it is $399, at 7.5 mg $499, and at 10 mg and above $699 per month. The Zepbound® label lists 5 mg, 10 mg and 15 mg as the maintenance doses, so a patient on the branded vials at a maintenance dose pays $399 to $699 a month against a flat $229, or $175 on Bodi Envi's twelve month plan. That is the comparison that matters over a course of treatment, and it is also a comparison between a compounded product and an FDA-approved one, which is the trade this whole guide is about.

Mounjaro® has its own savings card for commercially insured patients, which Lilly's Mounjaro® pricing page mentions, but because Mounjaro® is approved for type 2 diabetes rather than weight management it is not the natural comparison here and its terms are not repeated.

Personalized compounded medications differ from commercially available brand-name products in formulation, concentration, excipients, preparation methods, and other characteristics. ## Insurance coverage

Bodi Envi's programs are self-pay. Insurance is not required for applicable self-pay programs, and Bodi Envi does not bill health plans for them. That is all there is to say about insurance and Bodi Envi.

For the branded products the answer depends entirely on your plan. In general terms, a health plan decides whether it covers a medication, for which diagnoses, and under what conditions. Coverage of GLP-1 based medications for weight management varies widely between employers and insurers, and many plans that cover them require prior authorization, a request from your prescriber that the plan approves before it pays. Mounjaro® is approved for type 2 diabetes and Zepbound® for weight management and obstructive sleep apnea, and plans commonly treat those diagnoses differently.

No coverage percentages appear here because no source on this page publishes a reliable one. Lilly's own pricing pages say what this guide says: the amount you pay largely depends on your prescription drug plan, and the accurate answer comes from the plan. ## Prepaid terms and what they save

Bodi Envi publishes four terms for personalized compounded tirzepatide. The arithmetic between them is simple and it is shown in full.

The twelve month plan is $175 per month, which is $54 a month and $648 a year less than monthly billing. The three month plan saves $30 over its term and the six month plan $144. A prepaid plan is a commitment paid up front, and cancellation and refunds are governed by Bodi Envi's published refund policy, which sets out who is eligible, the time window for a written request, and what happens once medication has been dispensed. Read it before choosing a term.

There is a clinical reason to think in months as well. The Zepbound® prescribing information describes a dose escalation in four week steps, and the SURMOUNT-1 trial that studied the branded product ran for 72 weeks with a 20 week dose escalation period. A provider taking a similar approach with compounded treatment works over months, and the monthly price stays flat across all of them whatever your dose.
