AMONG OTHER ACTIVITIES, BODI ENVI’S SERVICES ENABLE COORDINATION AND COMMUNICATION WITH A HEALTH CARE PROVIDER. IT DOES NOT REPLACE YOUR RELATIONSHIP WITH ANY PHYSICIAN. THESE SERVICES MIGHT NOT BE APPROPRIATE FOR ALL MEDICAL CONDITIONS OR CONCERNS. IF YOU HAVE A MEDICAL EMERGENCY, IMMEDIATELY CALL YOUR DOCTOR OR DIAL 911.

Introduction
BODI ENVI, LLC, and their respective affiliates (collectively, “Bodi Envi”, “we”, “us”, or “our”) own and operate the websites located at BodiEnvi.com (collectively, the “Websites,” and collectively with any affiliated mobile application (“App”), the “Sites”). The Sites are intended to facilitate the provision of the Services to registered users.
The “Services” may include:
(i) providing individuals with information on health care and wellness (“Content”);
(ii) providing individuals with pharmacy services;
(iii) providing individuals with access to technology-oriented tools for health and wellness;
(iv) development and gathering of health care records and health care information with retention of the same for use in health care provider appointments, communications, and pharmacy services;
(v) administrative support in connection with scheduling and payment for health care provider and pharmacy services; and
(vi) telecommunications support for using the Services as a means of direct access to health care providers provided by Wasef Health, P.A. and similar affiliated professional entities for communication, consultations, assessments, and treatment by such health care organizations and their providers.

The Privacy Policy and the Bodi Envi Professionals Notice of Privacy Practices are hereby incorporated into these Terms of Use by reference and constitute a part of these Terms of Use.
Your Relationship With Bodi Envi
Bodi Envi does not provide medical services. Bodi Envi provides a technology platform that enables you to access a health care provider employed or contracted with Wasef Health, P.A. or similar affiliated professional entities.
The health care providers, not Bodi Envi, are responsible for all medical services provided to you. The decision to provide diagnosis, treatment recommendations, or prescriptions rests solely with the licensed health care provider.
You understand that by coordinating and, in certain cases, consulting with a Wasef Health, P.A. or affiliate health care provider through the Services, you are not entering into a provider-patient relationship with Bodi Envi.

Except for specific communications received from Wasef Health, P.A. or affiliate health care organizations and providers, none of the Content you receive through the Sites should be considered medical advice.
Ownership of Information Submitted via the Sites
With the exception of personal data governed by applicable privacy laws, any information you submit to Bodi Envi through the Sites or Services shall become the property of Bodi Envi and/or:
(i) Wasef Health, P.A.
(ii) similar affiliated professional entities, or
(iii) individual health care providers utilizing the Services.
Assignment
Wasef Health, P.A. and their affiliates may transfer contractual rights and obligations pursuant to these Terms of Use in connection with a merger, sale of assets, or similar transaction involving Bodi Envi.
